Solution
Set-up a regular phone and video call with Robert’s family and connect with staff in the facility to learn how to participate in various activities and social events to feel more connected with others.

Challenges
Sarah’s parents are not very tech savvy and unable to use the smartphone or computer to communicate to her. They are also experiencing declining health, which makes it difficult for them to speak for extended periods of time.
Solution
Sarah can work with the facility care staff and seek help in setting-up a regular phone and video call with Sarah’s parents. Also, request for them to provide status of their well being and any changes in their health.

Patient’s family who wants to keep them constantly updated with loved ones health status is challenging due to multiple activities and responsibilities required during work.
Solution
Zoey will help residents to stay in touch with their families by facilitating phone calls, video chats, and emails. She also provides regular updates on the residents' medical well-being to their loved ones. Remind residents on upcoming events & program activities offered by the facilities.
